Important Information About This Service:
The Library is very disappointed to announce that we can no longer offer passport services to the public effective February 12th. We have been informed by the U.S. Department of State that they incorrectly identified many non-profit and non-governmental organizations as eligible to operate as passport facilities, and they are now taking corrective action to rectify that. We sincerely apologize for this inconvenience but unfortunately it is out of our hands. The Bellport Post Office will continue to process passports for residents.
Passport processing is by appointment only. Please fill out the form below or call 631-286-0818 extension 1 to have a librarian fill out the form for you. You must have an appointment date and time confirmed by one of our passport agents. Absolutely no walk-ins. Passport Facilities only process new applicants. If you qualify for renewing your passport, you must mail in your application.
Our Passport Application Acceptance Program only serves applicants submitting form DS-11, such as first-time applicants, minor applicants, adult applicants whose most recent passport was issued as a minor, and applicants with a lost, stolen, or damaged passport. If you qualify for renewing your adult, full-validity passport on form DS-82, you must mail in your application following the instructions provided on that form.

The Passport Agency now has an online renewal service.
The library will also have paper copies of all applicable forms. Our fee for a passport photo is $10.
